Civvie Stuff
Name: Your characters name.
Age: How old your character is.
Sex: Male or Female? or Hermaphrodite? Whatever works for you.
Nationality: Where your character is from.
Occupation: How does your character earn his or her crust?
Species: Is your character Human? Fangire? Silurian? or something else?
Face Claim: Who you'd want to play your character if this was an actual TV Series? please check Here before committing.
Hair color/style: A short description of your characters hairstyle and color.
Eyes: A short description of your characters eyes, what color they are, whether they have glasses, etc.
Description: At least a paragraph to sum up your characters appearance, skin, size, clothes whatever.

Rider Stuff
Name: The name of your Kamen Rider, if you're using a canon rider check Here to make sure it isn't taken.
Henshin Device/Sequence: What your character uses to transform and what happens when it's activated.
Forms: A list of your characters forms when they're a Kamen Rider. If your character has Mix and Match powers (like Ozu or Double) then just list each individual component, don't go through every single combination, it'll take too long and you'll hurt your hands.

• Form Name: The name of the form.
  Appearance: A description of what it looks like, try and include a picture if you can.
  Transformation: How the form is activated and what the transformation into that form looks like.
  Weapons and Abilities: The forms weapons and abilities, please try and include pictures if you can find any.
